Grading Policy & Procedures

Grading System: NO COURSE GRADES WILL BE ROUNDED.

The clinical course grade is derived from clinical performance and written assignments. Students are also required to demonstrate math competency. Clinical performance, written assignment sections, and the math competency exam must be successfully completed in order to pass the course. Failure to pass the clinical course with a minimum grade of 75 will result in dismissal from concurrent courses and the nursing program.

Clinical Performance: is based on completion of all sections of the clinical evaluation tool, the ability to perform skills for the current clinical level as well as all skills from previous clinical levels, and professional behavior in the clinical setting.

(See the Clinical Points Tool form)

Written Assignments: All written assignments listed in the syllabus as well as any additional written assignments given by the clinical instructor must be satisfactorily completed and submitted on or before the assigned due date to the clinical instructor in order to receive a grade on the assignment. Failure to complete and submit assignments by the due dates will result in the student receiving a zero (0) for the assignment. All assignments must be completed and submitted in order to pass the course.

Math Competency:  Dosage calculation homework is assigned each week of clinical.  Clinical Nursing II students are required to take one dosage and calculation test.   The student will have two opportunities to pass the test with a minimum score of 90.  The student may use a calculator.  If the student is not successful on the first attempt he/she will be required to attend remedial math instruction and retake the math test within two weeks after the first math exam.    The student will not be able to administer medications in clinical until he/she has passed the math competency examination. Failure to pass the second math test with a minimum score of 90 will result in a clinical failure and dismissal from concurrent courses and the nursing program.

Performance Evaluation

Each student will be required to check-off on a randomly chosen nursing skill at the end of the semester as part of their end of semester performance evaluation.  Failure to successfully pass that skills check off will result in the required remediation with the skills lab coordinator and checking off on ALL previously learned skills.  Failure to successfully perform all skills will result in failure of the course.  The student has three attempts.

The following selected assignments and the performance evaluation will be graded to determine an overall numerical grade for written assignments in this course:

Percentage of overall numerical grade for Written Assignments and Performance Evaluation

Daily Care Plans                                                                         30%

Cultural  Quiz                                                                             10%

Care Plan Map Paper                                                                  20%

Performance  Evaluation                                                             20%

Professionalism Presentations                                                     20%

There will be four sim-chart assignments due throughout the semester.  These will be a completion grade only.  (See Clinical Points Tool).

The numerical course grade, after points are deducted for absences and other infractions  (see Clinical Points Tool), will be converted as follows:

                                                     Numerical Grade                            Letter Grade

                                                90 – 100                                           A

                                                81  -  89                                            B

                                                75  -  80                                            C

                                                66  -  74                                            D

                                                            65 and Below                                   F

If a Clinical Warning is issued, the final clinical grade will be reduced by one

letter grade.

Written Assignments – 40%

                                   +

Clinical Performance – 60%

                                      100% Total grade

RNSG1462 and RNSG 2404 are concurrent corequisite courses.  A failure in RNSG1462 or RNSG2404 will require the repetition of the other concurrent corequisite course. Each course will be graded separately.  Students must successfully complete each course with an average grade of 75 or above to progress to the next level or to graduate in the final semester. Unsafe clinical practice will result in a clinical failure.  If the grade is below 75 in RNSG 2404, the student will have to repeat RNSG1462 concurrently upon readmission.

Clinical Assignments

The student’s clinical rotation schedule will be distributed by the instructor at the beginning of the semester.  The schedule may be changed on an “as needed” basis by the instructor upon notification of the student.

Clinical Expectations

In order to meet the course objectives, the student will provide direct patient care for patients with health care needs, disease processes, illnesses and health education needs relating to nursing interventions specific to medical-surgical, obstetrical and pediatric care problems.  Specific clinical expectations will be given to the students during the orientation to the clinical agency.  Failure to meet these expectations will result in points taken from clinical performance grade (see Clinical Points Tool).

Clinical Evaluation

Each clinical assignment must be successfully completed and submitted for evaluation on or before the date it is due.  See attached Clinical Evaluation Tool, Medication Administration Policy and the NCTC Nursing Student Handbook for additional information related to the clinical evaluation.

  • Workplace Competencies

  • Resources: A worker must identify, organize, plan, and allocate resources effectively.  This includes (i) time, (ii) money, (iii) material and facilities, and (iv) human resources.

  • Interpersonal Skills: A worker must work with others effectively to (i) participate as a member of a team, (ii) teach others new skills, (iii) serve patients/customers, (iv) exercise leadership, (v) negotiate, and (vi) work with diversity.

  • Information: A worker must be able to (i) acquire and use information, (ii) organize and maintain information, (iii) interpret and communicate information and (iv) use computers to process information.

  • Systems: A worker must understand complex interrelationships as in (i) understanding systems, (ii) monitor and correct performance, and (iii) improve and design systems.

  • Technology: A worker must be able to work with a variety of technologies, g. (i) select technology, (ii) apply technology, and (iii) maintain and troubleshoot equipment.

Assignments Required in Clinical Nursing II

A Clinical Nursing Assignment Kardex must be filled out for patient cared for each week on the medical-surgical unit as well as ICU.  You will get your patient assignment the day of clinical and will be allowed to work on this paperwork throughout the clinical day when you are not providing patient care.

Four pediatric sim chart case studies.

A clinical log describing the students experiences - what was learned and how the student met the clinical objectives – to be completed on all units except the medical and surgical units and ICU.

Preceptor Feedback Sheets any time the student is assigned to a precepted area.

A group professionalism presentation to be assigned by the instructor and presented to the class.

Six care plans to be completed and uploaded in canvas within one week of patient care.

Math Exam – must be passed with a grade of 90 within two attempts or the student will receive a clinical failure, be dropped from the course, and dismissed from the nursing program.

Written mid-term self-evaluation, and written final self-evaluation.

Culture quiz that will be completed on canvas.

One Care Plan Paper to include: nursing kardex, and pathophysiology of medical/surgical condition.  Refer to Care Plan Paper grading criteria & instructions for preparing the Care Plan Paper.

Additional written work as assigned by the clinical instructor.

Criteria for Safe Clinical Performance

Patient safety is of paramount importance in the delivery of patient care. Therefore, it is necessary for the student and the nursing faculty to conscientiously assure safe nursing care behaviors. With supervision, the student will demonstrate the three roles of the nurse (Provider of Care, Manager of Care, and Member of the Profession) using the nursing process in meeting human needs.

SAFE CLINICAL BEHAVIORS INCLUDE BUT ARE NOT LIMITED TO:

UNSAFE CLINICAL BEHAVIORS INCLUDE BUT ARE NOT LIMITED TO:

1.

Provides for the physical care and safety of the patient.

Violates or threatens the physical safety of the patient (e.g., neglects use of side rails; restraints; comes unprepared to clinical).

2.

Maintains and monitors for the microbiological safety of the patient.

Violates or threatens the microbiological safety of the patient (e.g., violates aseptic technique, reports to clinical with personal illness).

3.

Maintains and monitors for the chemical safety of the patient.

Violates or threatens the chemical safety of the patient (e.g., violates the “5 Rights of Medication Administration”; fails to monitor IV infusions and/or patients’ responses to medications).

4.

Maintains and monitors for the thermal safety of the patient.

Violates or threatens the thermal safety of the patient (e.g., burns patient with hot packs, heating lamp, etc.; fails to observe safety precautions during O2therapy).

5.

Assures the psychological safety of the patient by using therapeutic communication.

Violates or threatens the psychological safety of the patient (e.g., uses non-therapeutic techniques repeatedly; attacks; derogates individual’s beliefs or values, provides inaccurate or incomplete information repeatedly in patient teaching).

6.

Accurately and/or adequately uses the nursing process.

Inadequately and/or inaccurately uses the nursing process (e.g., fails to observe and/or report critical data in reference to patients; repeatedly makes faulty judgements/decisions in nursing situations).

7.

Demonstrates the principles/learning/objectives in carrying out nursing care skills.

Violates previously mastered principles/learning/objectives in carrying out nursing care skills (e.g., unable to give injections; fails to obtain accurate vital signs).

8.

Assumes appropriate independence in action. Manages required patient care load at each course level.

Assumes inappropriate independence in actions or decisions (e.g., performs competencies not yet tested). Repeatedly unable to manage the required patient care load (e.g., fails to seek help in emergency situations).

9.

Seizes opportunities for growth in practice and recognizes personal strengths and limitations.

Fails to recognize personal limitations or incompetency (e.g., refuses to admit errors noted by instructor/nursing staff; places patient in the threatening or personal injury position, provides patient care while instructor not in clinical facility).

10.

Recognizes and accepts moral and legal responsibility for action.

Therapeutic Nursing Interventions

Therapeutic Nursing Interventions are acts carried out by nurses to foster and enhance the well-being of patients based on relevant scientific findings that guide a holistic approach to health care.  At the Clinical I and Clinical II Level of clinical performance, students are expected to demonstrate competency of selected psychomotor skills.  By the end of Clinical III mastery of these basic skills is expected.  Additionally, at the Clinical III and IV Levels of clinical performance, students are expected to demonstrate cognitive knowledge of the theory for skills performance that requires critical decision making even though they may not be able to demonstrate clinical mastery.

NORTH CENTRAL TEXAS COLLEGE

Clinical Site Medication Policy and Procedures

LEVEL - Clinical II and Transition Clinical

In order to maintain safety of the patient when students administer medications in contracted hospitals, the faculty at North Central Texas College ADN program has developed the following policies and procedures.

  1. Policy I
  2. The staff nurse has the final responsibility for patient's care.
  3. The clinical instructor or RN preceptor (instructor) has the final responsibility for student medication administration (supervision and delegation of experiences)
  4. This medication policy will be subject to the policy of the clinical facility.

Procedure:

The instructor will evaluate the student's assignment for the shift and assess patient, time factor, instructor availability and possible collaboration of a staff nurse. The instructor's decision to delegate the responsibility of medication supervision will be communicated directly to the staff and/or Charge Nurse.

The instructor's discretion will be used as to the student's readiness to do a skill involving medications. While North Central Texas College ADN program appreciates and values the collaboration of hospital staff nurses in this venture, it does not assume that staff nurses will comply. Collaboration will be sought when the instructor cannot be present and only by prior arrangement.  The nursing instructor will request communication concerning agency medication administration policy during student orientation.

  1. Policy II

When a staff nurse has agreed to supervise a student with the Clinical Instructor's discretionary permission, it is requested that the staff nurse check the poured medications for accuracy (the five rights) before the student can administer the medication to the patient. The student is to state the classification, purpose, common side effects, contraindications, and nursing implications of the medications(s) before administering to the patient. The student will document on the medication records and nursing notes ONLY medications they have administered; never prior to actually administering the medication to the patient.

General procedures to be followed:

  1. The students are permitted to administer the following routes of medication as noted in this policy. All medication administration by Clinical II students will be supervised. Clinical II students DO NOT administer IV push medications or any IV/ saline lock flush with heparin.
  2. Oral medications may be given with direct supervision of a staff nurse with approval of the Clinical Instructor.

¯  NOTICE

When Classes are CANCELLED

DUE TO BAD WEATHER

Many students commute to the NCTC campus in Gainesville from out of town, so hazardous driving conditions due to weather can be a problem.  Whenever the weather gets bad enough (icy roads, etc.) that you think NCTC might cancel classes, tune in to one of the radio or TV stations listed below.  These are the ONLY stations that will be notified.  By the way, if you don’t hear or see a notice about NCTC classes right away, be patient.  Please refrain from calling the station because it ties up the very busy lines, and the college may be trying to get through with a cancellation notice!

LionAlert is the easiest and earliest way to find out about campus closings.

Go to the NCTC home page and click on the LionAlert icon to sign up for free emergency text messaging service. In the event of an emergency or major campus event, a short message will be sent to the mobile number and/or email address provided.

If you miss the announcements on the air, you can call the campus switchboard at (940) 668-7731.  On bad weather days, there will be a recorded message beginning at 6:45 AM announcing any class cancellations.

IMPORTANT:  Class time missed as a result of unanticipated class cancellations due to weather MAY HAVE TO BE MADE UP.  If so, you will be notified by the Office of the Dean of Instruction.

If classes are dismissed on the NCTC campus due to bad weather, clinical will also be dismissed.

If classes are not dismissed on the NCTC campus and driving conditions in the clinical areas are hazardous due to weather, the clinical instructor and the student will make the decision about safe driving conditions.

Guidelines for Care Plan Paper

  1. The care plan paper must be accompanied by document of a patient assessment on the nursing kardex form. You are not to make-up any data.  You are expected to look up any and all data on the patient's chart/record.  If the patient has a lot of lab values, you may wish to make a chart to show the trending of the lab values.
    1. Form must be complete
    2. Labs are analyzed and related to the patient’s condition
    3. Medication list is complete with the reason for administering them (all medications).
    4. Drug cards are completed for each medication the patient is taking (up to 10)
  2. Medical diagnosis, comorbidities and pathophysiology of current medical problem to include:
    1. A thorough explanation of the pathophysiology of the primary diagnosis starting at the cellular level.
    2. Clinical manifestations that is associated with the diagnosis and the physiological basis for these manifestations.
    3. How this disease process is manifested in your patient.
    4. A brief explanation of all the patient’s co-morbidities and how they relate to the current diagnosis.
    5. Typed, grammatically correct, and references must be listed using at least three sources: peer reviewed journal articles, textbooks, patho book, etc. You may also use the internet.  You must use at least two journal articles with at least one being a full text source from CINAHL.  If you do not know how to access the Nursing Collection on CINAHL, you need to visit the NCTC library and learn how.
  3. Your nursing diagnoses must be in NANDA format.  You must also indicate how you would prioritize them using Maslow’s Hierarchy.  List or number your nursing diagnoses in order to indicate priority.  List the 5 most important nursing diagnoses for your patient. One of the nursing diagnoses should be psychosocial. Complete care plans on the two highest priority nursing diagnoses.
    1. Nursing diagnoses must be placed in order of priority.
    2. An explanation of how the diagnoses were prioritized using Maslow’s Hierarchy of Needs in addition to any other criteria (i.e. ABC’s).
  4. The goal statements (Planning).  This is done for the two top nursing diagnoses you are to complete concept maps on AND write about in your paper.
    1. There is a general goal statement that includes the resolution of the (nursing diagnosis).
    2. There are expected outcomes that will measure the patient’s progress toward or allow the patient to achieve their goal (s) that are
      1. Specific to the patient
      2. Measurable
  • Have a time limit
  1. Realistic
  2. Written in terms of patient behaviors or findings.
  1. Nursing interventions (Implementation) are directed toward reducing or removing stated problem (nursing diagnosis) or to achieve goal criteria (expected outcomes)
    1. The interventions are at a level of caring for the patient with common healthcare needs
    2. The interventions incorporate patient or family teaching
    3. The interventions include (but are not limited to):
      1. Medication administration and monitoring for side effects
      2. Nursing assessments and interpretations of the findings
  • Performance of procedures or treatments
  1. Patient advocacy is indicated.
  2. Monitoring and interpreting laboratory results
  3. Psychosocial assessment and intervention.
  1. Nursing actions must also be written in very specific terms.  For example, “Administer 50mg Demerol IM every 4 hours as needed for pain.”  Or, “Consult with physician to obtain an order for a bulk laxative.”  Regardless of which resources you use, you need to consider including actions that have to do with assessment needs, physical nursing interventions (such as wound care, elevation of extremities etc), food and fluid needs, medications, teaching needs, and appropriate referrals.  Do not put Administer whatever it is “as ordered”. Always specify how it is ordered.
  1. Each nursing action (Intervention) is supported by a rationale
    1. The rationale explains how the intervention will help resolve the problem (nursing Dx) or help the patient achieve their goals.
    2. Each rationale is documented by nursing resources that include
      1. The author and the page number or
      2. Web address for the source (must be a credible source such as ANA, CDC.gov, etc.
    3. A variety of resources are used to include (at least two journal articles with at least one being a full text source from CINAHL).
      1. Textbooks
      2. Care plan books
  • Journal articles (current, within the last 5 years)
  1. Internet searches
    1.  A bibliography must be included listing all references using APA format.  Access the Purdue Online Writing Lab for assistance (no points will be deducted for incorrect format)

https://owl.english.purdue.edu/

  1. Evaluation of the patients’ goals includes statements as to the patient’s progress toward achieving their goals.  Each expected outcome is evaluated by:
    1. A statement as to the patient’s progress toward achieving that outcome (achieved, not achieved, or partially achieved)
    2. What findings made to determine whether the criteria was achieved or not.
    3. Any revisions in your plan that might be needed or whether the problem is resolved.
  2. The concept map paper must be turned in on time with correct spelling, legible and citation of sources with the grading criteria formed attached.  All work must be neat, in ink (data base only) or typed.

**This is a formal paper.  The information required must be written out in a narrative form.  Above all, please remember that your paper will be graded according to the grading criteria sheet.  (For Example:  Even if you do an absolutely outstanding job on everything else, but you fail to provide a thorough pathophysiology explanation, you will not receive the points allocated for the pathophysiology explanation.)  You must fully meet the grading criteria to receive full credit

Students will be assigned to groups within your clinical groups and assigned a professional topic to develop a presentation that will be given in post conference on a date to be determined by your instructor.

Provide a list of the objectives, an outline of the content, and a list of the references to the faculty prior to presentation (can be the day of the presentation).  Power points presentations are acceptable but not required.  Power point slides should be readable with large font and not too many words on one slide.  Please do not read the slides

PEER EVALUATIONS:

Each member of the group must participate in the planning and the presentation of the topic (please see and use attached form). Each member will be able to evaluate the contribution of other group members, which will reflect the percentage of the grade received.  If group members evaluate your contribution as 100%, you will receive 100% of the grade earned by the group.  If group members evaluate your contribution at 90%, you will receive 90% of the grade earned by the group, etc.

Please use the attached form for group member evaluations.  These are to be turned in the day of the presentation to calculate grades.

POLICY FOR SOCIAL NETWORKING SITES

Students in the Associate Degree Nursing Program at North Central Texas College are expected to adhere to the high standards of the nursing profession with regard to maintaining confidentiality.  This not only includes guarding patient confidentiality at a clinical site, but also in the classroom, at home and on-line.

The following are guidelines for behavior involved with cell phone use, FACEBOOK, Twitter, MySpace and any other social networking site.  Many of you already have such sites established and are eager to use them to convey what you are learning and experiencing while in this program.  You may continue to use those sites, but with these cautions:

  • It is your responsibility to keep your site appropriate and your profiles clean.
  • Do not post threats or derogatory remarks about anyone associated with the Associate Degree Nursing Program.  This includes fellow students, faculty, staff, college administrators, clinical affiliates and, above all, patients.   This is a violation of “good professional character” policies described in Texas Board of Nursing Rule §217.11 and Associate Degree Nursing Education Rule §215.8 (“student dismissal for lack of good professional character”).   Any threat of bodily harm may be reported to the police and violation of this policy will result in disciplinary action, such as dismissal.  Additionally, those who violate these rules may be reported to the Board of Nursing.
  • Any photos of yourself must be made out of uniform, make no reference to NC TC, the Associate Degree Nursing Program or our clinical affiliates, and will not include any illegal activity.  Posting photos of other students, faculty, staff, etc. without their permission is forbidden.
  • Cell phone use in the classroom is forbidden and at the clinical site is dependent on clinical facility policies and only with instructor permission.   If a phone rings during the classroom lecture or during clinical in a facility that forbids the use of cell phones, the phone will be confiscated until the end of the class or clinical day.
  • Computers in the classroom: may only be used for taking notes of the lecture or for internet searches pertinent to the current lecture content.  If the instructor determines you are emailing, surfing the web for personal issues, or accessing social media sites such as Facebook, you will not be allowed to bring your computer to class.

My signature acknowledges I have been informed of the Social Networking Policy for the Associate Degree Nursing program at North Central Texas College.

RECEIPT OF SYLLABUS

I have received a copy and understand the syllabus for RNSG1462 Clinical Nursing II.  The items in the syllabus have been explained to me.

I understand that the instructor reserves the right to drop students due to excessive absenteeism.

I understand that if I stop attending the course and I fail to drop this course through the registrar's office, I will receive a letter grade of "F" in the course. I understand that NO GRADES ARE ROUNDED IN THIS COURSE.

The requirements of the course have been explained to me.

I understand that I must pass the required Math exams with a grade of 90. I will have two attempts to pass the exams. If I do not pass the exam with a 90 on the 1st or 2nd try, I understand I will receive a clinical failure, be dropped from the course, and dismissed from the nursing program.

Students are expected to attend clinical on each assigned clinical day.  There will be no clinical absences.  If an untoward circumstance arises and the student must miss a clinical day, three (3) points will be deducted from the final grade for each clinical day missed.

Penalties for tardiness will be imposed at the discretion of the instructor.  However, repeated tardiness is not acceptable and may result in the loss of a letter grade in the course.

I realize that I may receive a clinical warning for exhibiting unsafe clinical behavior, inadequate clinical preparation, unprofessional conduct, or for not notifying the clinical instructor of a clinical absence or delay in arriving to clinical in a timely manner.  (See Clinical Warning in the Student Handbook.)  A clinical warning will result in the loss of one letter grade for the course.  Unsafe clinical behavior or unprofessional conduct may, at the discretion of the faculty, result in a clinical contract or clinical failure.

I have read the college’s guidelines concerning the disclosure of disabilities.  I understand the guidelines and I know that it is my responsibility to tell my instructor if I have special learning needs.

I give permission for photographs and videos to be taken of me in class, lab, or clinical to be used in teaching and/or in college promotional materials.

The instructor has my permission to post my grades on a password protected internal site.

The Nursing Department has my permission to post my name on clinical lists, class rosters, and other lists as deemed necessary for students enrolled in the Associate Degree Nursing program.

I give permission for the Associate Degree Nursing Department to provide my criminal background check and drug screening information to all agencies and facilities used as clinical rotation sites during the semesters I am enrolled in the ADN Program.  I understand that clinical agencies and facilities have the right to refuse my attendance based on the information contained in the criminal background check and drug screen.  I understand that I may not be able to complete the ADN Program clinical requirements resulting from agencies and facilities refusing my clinical attendance.  I understand that the Board of Nursing may refuse my application for licensure based on the information contained in the criminal background check and drug screen.
