Development of skills and techniques required for a comprehensive nursing health assessment of patients across the lifespan. Includes assessment of patients' health promotion and maintenance, illness and injury prevention and restoration, and application of the nursing process within a legal/ethical framework. This course lends itself to either a blocked or integrated approach.

STUDENT LEARNING OUTCOMES (From Academic Course Guide Manual/Workforce Education Course Manual/NCTC Catalog

At the successful completion of this course the student will be able to:

 

Demonstrate techniques utilized in the nursing health assessment of individuals across the lifespan

 

Perform systematic physical assessment using appropriate equipment and techniques for all body systems

 

 Utilize cognitive and psychomotor skills in assessing patients' physiologic health and psychosocial status

 

Document findings utilizing appropriate terminology and legal/ethical format

 

Identify legal and ethical standards related to health assessment

 

The course has an online lecture component and a virtual and on campus lab component. Unit exams will be proctored through ExamSoft and comprised of questions from both the lecture and the lab. Assessment techniques will be demonstrated online through ATI Health Assess and on campus labs. Student will be routinely evaluated in virtual lab and on campus lab to determine capability of completing the assessment correctly. NO GRADES WILL BE ROUNDED IN THE COURSE. (Example 74.9 is less than 75 and will result in receiving a D for the course.

 

In order to pass the course with a grade of C or higher, the student:

  1. Must pass all course assignments with an average of 75 or higher.
  2. Must successfully meet all the requirements of the lab component. (Requirements for the lab component may include HealthAssess modules, Virtual Practice/Challenges, assessment demonstrations and participation in virtual lab activities)
  3. Must be able to perform a head to toe physical assessment on a patient.
  4. If we are unable to come to campus for any reason we will move to all virtual content. You will do a virtual assessment, but only if we cannot come to campus.

 

We will have a vital sign check off. If you cannot complete the vital sign check off you will not be allowed to go to clinical. You will incur point deductions for each day missed from clinical for failure to complete the Vital Sign Checkoff.

The numerical course grade will be converted as follows:

A grade of “C” or better (75%) is required in this course to progress within the nursing program.

ATTENDANCE POLICY

Regular and punctual attendance is expected of all students in all classes for which they have registered.  All absences are considered to be unauthorized unless the student is absent due to illness or emergencies as determined by the instructor.  It is the student responsibility to provide documentation as to the emergency for approval and judgement by the faculty member.  Approved college sponsored activities are the only absences for which a student should not be held liable and only when provided by a college official ahead of the absence.  Valid reasons for absence, however, do not relieve the student of the responsibility for making up required work.  Students will not be allowed to make up an examination missed due to absence unless they have reasons acceptable to the instructor.  A student who is compelled to be absent when a test is given should petition the instructor, in advance if possible, for permission to postpone the exam.  Student will be dropped from a class by the Registrar upon recommendation of the instructor who feels the student has been justifiably absent or tardy a sufficient number of times to preclude meeting the course’s objectives.    Persistent, unjustified absences from classes or laboratories will be considered sufficient cause for College officials to drop a student from the rolls of the College. Last day to withdraw from a course with a “W” is April 5, 2021.

 

DISABILITY SERVICES (Office for Students with Disabilities)

The Office for Students with Disabilities (OSD) provides support services for students with disabilities, students enrolled in technical areas of study, and students who are classified as special populations (i.e. single parents).

Support services for students with disabilities might include appropriate and reasonable accommodations, or they may be in the form of personal counseling, academic counseling, career counseling, etc.  Furthermore, OSD Counselors work with students to encourage self-advocacy and promote empowerment. The Counselors also provides resource information, disability-related information, and adaptive technology for students who qualify.

Consequences for academic dishonesty may include:

In the best interests of our students’ health, and to ensure consistent access to your classes throughout the semester, NCTC moved the majority of courses for the Fall semester online, or as synchronous online which includes weekly, scheduled virtual lectures with your instructor. The Associate Degree Nursing classes will require in-person instruction offered on campus by practicing required safety and social distancing guidelines. NCTC faculty and staff will continue to offer the instruction and support you need to be successful as we assess our campuses as we reopen to the public, and all of our Student Services’ offices are working to meet your needs.

The Associate Degree Nursing takes COVID 19 very seriously. All students must complete

  1. Complete the Daily Health Check prior to going to clinical or campus. The link can be found here https://www.nctc.edu/coronavirus/index.html
  2. Must wear a mask on campus.
  3. Must follow the rules for COVID 19 clinical facility has enacted.

Even if it is more stringent than NCTC’s.

  1. When on campus please park with a space between cars.
  2. You must enter at the front of the HSC building.
  3. All faculty will wear masks.
  4. Social distancing will be enforced while students are on campus.
  5. There will be no congregating in the buildings.
  6. Only 2 students at a time are allowed on the elevators, with masks on.
  7. If you test positive to COVID-19 you may report the information to the Office of Emergency Management by emailing Patrick Abu at pabu@nctc.edu.
  8. You should stay at home except to get medical care.
  9. If you have confirmed or suspected COVID-19 and have symptoms, you can stop your home isolation when:
    1. You’ve been fever-free for at least 24 hours without the use of fever-reducing medication AND
    2. Your symptoms  have gotten better, AND
    3. At least 10 days have gone by since your symptoms first appeared
  10. If you tested positive for COVID-19, but have not had any symptoms, you can stop your home isolation when:
    1. At least 10 days have gone by since the date of your first positive COVID-19 test, AND
    2. You have not gotten sick with COVID-19
  11. Instructions after completing self Isolation
    1. Report that your quarantine period is over to The Office of Emergency Management by emailing Patrick Abu at pabu@nctc.edu.
    2. Wait for approval to return on campus. The Office of Emergency Management will provide additional information and approval.
    3. NCTC may require clearance from a healthcare provider to return to class located at an NCTC campus, if applicable.

Students who test positive for COVID-19 may return to NCTC classes and activities after testing negative and after their return has been cleared by their health care provider or in accordance with the most recent local public health and medical guidance. Proof of a doctor's release may be required before you are allowed to return on campus.

Clinical learning experiences shall include the administration of medications, health promotion and preventive aspects, nursing care of persons across the life span with acute and chronic illnesses, and rehabilitative care.

(1) Students shall participate in instructor-supervised patient teaching.

(2) Students shall also be provided opportunities for participation in clinical conferences/debriefing.

(3) When a high-fidelity simulation laboratory is used to meet clinical learning objectives, the faculty shall be trained in planning and guiding the experience and in debriefing and evaluating students. Programs may use up to 50% simulation activities in each clinical course. This means you must have 50% of your clinical experience at the Bedside.

Students who miss 2 or more clinical days at the clinical site or on campus will be reviewed by the Clinical Instructor, Chair of the Program and the Dean of Health Sciences for completion requirements.

 

Formal NCTC COVID-19 Statement

 

Conversion of Onsite Classes to Online/Remote Format: North Central Texas College students should be aware that in the event of a college closure due to COVID-19, onsite classes will be converted to an online/remote format. Students should plan ahead to ensure they have access to the computer equipment (either PC, MAC, or tablet), webcam, and internet connectivity to continue their classes in an online/remote format. Please read all your official North Central Texas College student emails as the transition from onsite to online/remote might require a reorganization in your personal situation. Students will be granted a 72-hour transition and grace period. Online classes will continue as scheduled without disruption. Wear a mask, stay safe, and contact your Instructor as the situation arises. These policies and procedures were updated on July 30, 2020 and are subject to change as conditions change.

 

Face Coverings: Per the North Central Texas College guidance on face coverings on campus, in the instructional setting, faculty and students must wear face coverings, such as masks or face shields. Students without coverings, or those who do not comply with the rules relating to face coverings, will not be able to participate in on-campus classroom activities. To request an exception to this requirement, students should contact the NCTC HR Office of Enrollment Management (ccove@nctc.edu). Failure to comply with the face coverings requirement may result in the Instructor directing the student to leave the classroom. Any student asked to leave the classroom may be referred to the student conduct officer. These policies and procedures were updated on July 30, 2020 and are subject to change.

 

Temporary COVID-19 Attendance Policy for Face-to-Face Meetings: We are facing an unprecedented situation in which all of us must be flexible and make prudent decisions in the best interest of our families, our campus, and our community. In light of this, North Central Texas College is temporarily establishing the requirement that faculty keep records of student attendance for face-to-face course meetings as well as a documented seating chart. In addition, students who are sick or need to quarantine should not attend classes. Students will not be required to provide formal documentation from a health care provider and will not be penalized for COVID-19 related absences when proper notification to campus health officials is made in accordance with the guidelines stated below.

 

Faculty will:

 

  • Notify students about important course information and delivery changes through Canvas and campus email.

 

Students should:

  • Provide notification to campus officials (via NCTC Daily Health Check protocol through Canvas) if they have tested positive for COVID-19 or have to quarantine so we can confirm reported absence with instructors, monitor, and assist the campus community.
  • Notify instructors in advance of the absence.
  • Connect with that class through Webex if the class session is being transmitted in a hybrid fashion.
  • Keep up with and/or make up missed classwork or assignments.
  • Submit assignments digitally through Canvas or other means as announced by your instructor.
  • Work with their instructors to reschedule exams, labs, and other critical academic activities described in the course syllabus.
  • Check Canvas and campus email daily to receive important announcements pertaining to the course.

During the spring 2021 semester, faculty with face-to-face meetings will establish assigned seating/work stations to facilitate roll-taking, and, if necessary, contact tracing. Additionally, we ask all members of the College community to be attentive to their health, and safeguard others, by following the CDC’s guideline to “stay home when you are sick.” You should stay home if you have symptoms.
